2007-2008 Registration Form

The form to register for one of the WARP programs for the 2007 - 2008 ski season (Dec. 2007 through Apr. 2008) is included in the WARP brochure. 

Participants must sign the form in addition to a parent or guardian.  Don't forget to sign the Release Waiver on the back!
