2009-2010 Registration Form

The form to register for one of the WARP programs for the 2009 - 2010 ski season (Dec. 2009 through Apr. 2010) is included in the WARP brochure. 

Participants must sign the form in addition to a parent or guardian.  Remember to sign the Release Waiver on the back!
